Getting in

How to become an Interventional Gastroenterologist

The path is medical school, a three-year internal medicine residency and a three-year ACGME-accredited gastroenterology fellowship, followed by an additional advanced endoscopy fellowship, usually one year, focused on ERCP and endoscopic ultrasound.

The American Board of Internal Medicine certifies physicians in internal medicine and gastroenterology. The American Society for Gastrointestinal Endoscopy (ASGE) sets training standards for advanced endoscopy and lists fellowship programs. Gastroenterology fellowship is one of the more competitive internal medicine fellowships, and advanced endoscopy positions are fewer still. Applicants usually show a high volume of procedures during general GI fellowship, research in pancreaticobiliary disease and letters from advanced endoscopists.

The work

What does an Interventional Gastroenterologist do?

Interventional gastroenterologists use endoscopes to diagnose and treat complex digestive disease. They perform ERCP to clear bile duct stones and place stents, endoscopic ultrasound to sample tumors of the pancreas and nearby organs, and procedures that remove large precancerous growths or early cancers from the esophagus, stomach and colon without surgery.

They also drain fluid collections around the pancreas, treat swallowing disorders with endoscopic muscle cutting, and stop serious bleeding. Like other gastroenterologists, they see patients in clinic and perform routine colonoscopy and upper endoscopy, but their referrals come largely from other gastroenterologists and surgeons.

Day-to-day tasks

  1. Perform ERCP to remove bile duct stones and place biliary and pancreatic stents
  2. Perform endoscopic ultrasound and fine-needle sampling of pancreatic and other tumors
  3. Remove large polyps and early cancers with endoscopic resection techniques
  4. Drain pancreatic fluid collections through the stomach wall
  5. Treat achalasia and other swallowing disorders endoscopically
  6. Control upper and lower gastrointestinal bleeding
  7. See patients in clinic and coordinate care with surgeons and oncologists

Typical tasks, researched by NueCareer from the sources below.

Working conditions and hours

Interventional gastroenterologists work in hospitals, academic centers and large GI groups with endoscopy units and fluoroscopy suites. Days are procedure-heavy, with call for bleeding and bile duct emergencies. ERCP uses X-ray, so radiation protection is routine.
